I am hoping to survey students and teachers about how they use and perceive the library. I want to use the results to improve my program and for advocacy reasons. Has anyone done a user survey? Any advice? Sample questions would be very useful!

Doug's web page is very helpful. I have done similar surveys in the past. I have also run focus groups, asking similar questions. Although I have run blogs on other subjects, they should work as a means of getting feedback on usage and attitudes.
